While out on an oc-1 solo run, what item(s) would you recommend to have in case of trouble, i.e. paddle, iako, ama cracks or breaks? What has been your experience?

A cell phone in a waterproof bag for sure. You can get them West marine, island paddler or internet marine sites.

If you haven't checked out the Articles section, there are a few good articles written by Luke titled "An Introduction to OC1 Paddling" and another one by Skipper Rich titled "Outrigger Canoe Safety - What Every Paddler Needs to Know". There is a list of items that is most common to bring along just in case "Murphy's Law" comes into play and explains why and how they are used in situations.

Most of the time we jump in the water, we forget or don't have on hand all the recommended items, so we paddle without them most of the time. If this is the case, then I personally recommend that you acquire the ability, to at least, be able to swim a mile in half an hour or so. Doing so increases the probability of being able to handle the current if it is carrying you out to sea. Also, always swim in with the breakers or surf. Sure you'll get knocked around, but you're heading inside and not outside.

Cell phone, PFD, whistle and mirror. Tell someone where you're going and when you'll be back. I am forced to paddle solo alot around here since moving. Wish I didn't have to, but it is what it is...

I watched Rambo pull a roll of duct tape from somewhere on his canoe, maybe an orifice :0) and tape up a gaping hole in the hull after I stuck a Pegasus through the side of his ol Hurricane. They don't call him Rambo for nothing! I thought we were in deep shit being about three km offshore.
My advice, take Rambo with you......

The same Hurricane eventually got her revenge and T boned the said Pegasus a while back, no duct tape this time but was only 200m from land. I'm sure i heard my Hurricane laughing as we paddled in !!!

If you need to ask others 'what you should bring on a solo trip' you shouldn't be going alone. In addition to gear, intangibles like ability, skill, knowledge are required. My advice, stay close to a buddy and shore.
